Evaluación del PET-CT interim en pacientes con linfoma de Hodgkin. Experiencia en el Hospital de Clínicas “Dr. Manuel Quintela” y CUDIM. 2017-2023

Abstract

Este estudio evalúa el papel de la Tomografía por Emisión de Positrones-Tomografía Computada (PET-CT) interim en el tratamiento de pacientes con Linfoma de Hodgkin (LH) tratados con el esquema Doxorrubicina, Bleomicina, Vinblastina y Dacarbazina (ABVD) en el Hospital de Clínicas “Dr. Manuel Quintela” y valorados con PET-CT en CUDIM en los años 2017 a 2023. Se analizó una cohorte retrospectiva de 28 pacientes con diagnóstico confirmado de LH. Las imágenes de PET-CT fueron evaluadas en tres etapas: estadificación inicial, interim y final de tratamiento. Se utilizó el Deauville Score (DS) para clasificar las puntuaciones de 1–3 como negativas y de 4–5 como positivas. También se aplicaron parámetros semicuantitativos, como el Delta Standardized Uptake Value (DSUV) y el Índice Tumor Hígado (TLr), para examinar su valor y predecir la respuesta al tratamiento y la recaída. Los resultados mostraron que el PET-CT interim utilizando DS es un fuerte predictor de buena respuesta, con un valor predictivo negativo (VPN) del 95% en los casos negativos. El DSUV mostró una sensibilidad del 100% pero una especificidad menor (64%) en comparación con el DS. El análisis hematológico posterior a 2 ciclos de tratamiento reveló aumentos significativos en la serie roja y disminuciones en la serie blanca, sugiriendo efectos secundarios hematológicos. Los hallazgos respaldan el uso del DS como método primario para evaluar la respuesta al tratamiento en LH, mientras que el DSUV y el TLr pueden ser complementarios. Se requieren estudios adicionales con mayores cohortes para confirmar la utilidad de los métodos semicuantitativos en esta población.

This study evaluates the role of interim Positron Emission Tomography Computed Tomography (PET-CT) in monitoring treatment response in patients with Hodgkin's Lymphoma (HL) treated with the Doxorubicin, Bleomycin, Vinblastine, and Dexamethasone (ABVD) regimen at Hospital de Clínicas “Dr. Manuel Quintela” and CUDIM between the years 2017 and 2023. A retrospective cohort of 28 patients with confirmed HL was analyzed. PET-CT images were evaluated at baseline, interim, and end-of-treatment stages using the Deauville Score (DS), classifying scores of 1–3 as negative and 4–5 as positive. Semi-quantitative parameters, such as Delta Standarized Uptake Value (DSUV) and Tumor Liver ratio (TLr), were also applied to examine their predictive value for treatment response and relapse. Results showed that interim PET-CT using DS is a strong predictor of good response, with a negative predictive value (NPV) of 95% for negative cases. DSUV showed 100% sensitivity but a lower specificity (64%) compared to DS. Hematological analysis post-treatment revealed significant increases in red cell parameters and decreases in white cell counts, suggesting treatment-related hematologic effects. The findings support the use of DS as a primary method to evaluate treatment response in HL, while DSUV and TLr may offer complementary information. Additional studies with larger cohorts are needed to confirm the utility of semi-quantitative methods in this population.
