Adherencia a guías de tratamiento de la Neumonía Aguda Comunitaria en el Hospital de Clínicas julio/agosto 2019

Abstract

Importancia: En el 2018 se implementó una guía para el tratamiento de la Neumonía Aguda Comunitaria (NAC) en el Hospital de Clínicas “Dr. Manuel Quintela” (HCDrMQ), dentro del Programa de Optimización de Antimicrobianos (PROA). La aplicación de guías clínicas disminuye la morbimortalidad siendo fundamental su auditoría. Objetivo : Determinar el nivel de adherencia de los médicos a la guía desarrollada y el grado de apropiabilidad. Metodología : Se realizó un estudio descriptivo transversal en base a la revisión de historias clínicas de pacientes con NAC ingresados al hospital desde el 12 de julio al 6 de agosto del 2019 relevando datos del tratamiento antibiótico. Se realizó una encuesta online dirigida a médicos de la institución evaluando el conocimiento y percepción de apropiabilidad de la guía. Resultados: Fueron incluidos 24 pacientes. No se encontró registro de scores de gravedad en las historias. En el 92% el antibiótico fue acorde a lo recomendado. El switch precoz a vía oral se hizo en 1 paciente de 15 que cumplían las condiciones. Se realizó descalamiento ntimicrobiano en 1 paciente. La media de duración de tratamiento fue 11 ± 4.4 días, en 4 de 21 pacientes se cumplió la recomendación de 7 días. La encuesta la respondieron 51 médicos: 92% afirmó conocer la guía y 90% aplicarla. Discusión y Conclusiones: Aunque lo referido en las encuestas evidencia un alto nivel de conocimiento y apropiabilidad de la guía, el nivel de adherencia observado fue bajo. El antimicrobiano indicado fue la única recomendación que tuvo un nivel de adherencia mayor al 90%, con un manejo heterogéneo en el resto de las recomendaciones. Sin embargo, de la encuesta surge una percepción de los clínicos opuesta. El problema no impresiona ser la difusión de la guía, sino la aplicación de la misma.

Background : In 2018 a guideline for the treatment of Community Acquired Pneumonia (CAP) was implemented at the Hospital de Clínicas "Dr. Manuel Quintela" (HCDrMQ) within the Antimicrobial Optimization Program (PROA). The application of clinical guidelines decreases morbidity and mortality, and their audit is fundamental. Objective : To determine the level of adherence of physicians to the developed guideline and the degree of appropriability. Methodology : A cross-sectional descriptive study was performed based on the review of medical records of patients with CAP admitted to the hospital from July 12 to August 6, 2019, collecting antibiotic treatment data. An online survey was conducted aimed at physicians of the institution evaluating the knowledge and perception of appropriateness of the guide. Results : 24 patients were included. No record of severity scores was found in the medical records. In 92% of cases the antibiotic was in accordance with the recommendations. Early oral switch was made in 1 patient out of 15 who met the conditions. Antimicrobial de-escalation was performed in 1 patient. The mean duration of treatment was 11 ± 4.4 days, in 4 of 21 patients the recommendation of 7 days was met. The survey was answered by 51 doctors: 92% said they knew the guideline and 90% said they applied them. Discussion and Conclusions : Although the survey shows a high level of knowledge and appropriateness of the guideline, the level of adherence observed was low. The antimicrobial indicated was the only recommendation that had a level of adherence greater than 90%, with heterogeneous management in the rest of the recommendations. However, an opposite perception emerges from the survey. The problem is not the diffusion of the guide, but its application.
