Episodic future thinking is the human ability to imagine future events and project oneself into them, a crucial process for everyday planning and decision-making. The aim of this study was to conduct a systematic review and synthesize the scientific literature to date on the functioning of episodic future thinking in people with schizophrenia spectrum disorders, highlighting how these people project possible future events and what other phenomena it is related to. An exhaustive search for empirical studies was carried out using electronic databases, through the PRISMA method, and 16 relevant studies that met the inclusion criteria were selected. The results showed that people with schizophrenia have deficits in the functioning of episodic future thinking, which can be observed both in the difficulties in creating possible future scenarios and projecting oneself into them, as well as in the imagination of general, incoherent and poorly detailed future simulations. The studies also showed deficits in autonoetic awareness, autobiographical memory and not only episodic but also emotional foresight. Furthermore, these difficulties were found to be related to negative symptoms such as apathy and anhedonia, which reduce motivation to carry out daily activities. Only one intervention study was found, where life review therapy showed improvements in specificity and other aspects of episodic future thinking, although further research is needed to reach a solid statement in this regard. In conclusion, this review highlights the importance of episodic future thinking and the impact of its functioning in the daily life of people with schizophrenia, affecting their functionality and autonomy. Therefore, delving deeper into this topic and developing effective interventions that improve the future projection of these people is essential for them to achieve a better quality of life and greater autonomy.
