Introducción: El trasplante renal (TR) es el tratamiento de elección en la enfermedad renal crónica terminal. Las barreras al acceso oportuno a TR están vinculadas al paciente y al sistema de salud.
Objetivo: Conocer el tiempo del proceso de evaluación pre-trasplante del Centro del Hospital de Clínicas e identificar posibles factores de demora.
Metodología: Estudio observacional de cohorte, retrospectivo. Se revisaron historias clínicas, se incluyeron pacientes en evaluación pre-trasplante del Hospital de Clínicas entre el 1-01-2018 y 31-07-2021. Se definieron 2 grupos según la presencia de demoras en el proceso de estudio: con y sin inconvenientes.
Resultados: La mayoría de los pacientes (83,4 %) se incluyó en el grupo con inconvenientes en el proceso de evaluación; con un tiempo de estudio mayor que el grupo sin inconvenientes (11 vs 4 meses, p=0,000). La causa de demora más frecuente fue vinculada a factores del paciente (64%). La contraindicación temporal más frecuente fue la obesidad (54%). Los pacientes con prestador público en comparación al privado tuvieron una mayor proporción de inconvenientes (86,7% vs 75,9%, p<0.05) y el tiempo de referencia a pre-trasplante con respecto al inicio de diálisis fue mayor (mediana 11 vs 5 meses, p<0.05). Luego del 2020, disminuyeron el número de primeras consultas (sin diferencias significativas) y el número de pacientes con inconvenientes disminuyó (51% vs 86%, p=0.000).
Conclusiones: La mayoría de los pacientes presentó inconvenientes durante el proceso de evaluación pre-trasplante que retrasó su ingreso a lista de espera. El principal inconveniente detectado fue relacionado al paciente con inasistencia a consultas y/o dificultades en la coordinación de estudios. A futuro es importante diseñar estrategias que contemplen dichas dificultades y evaluar factores psicosociales y gestión del sistema de salud.

Patients with a public provider compared to a private provider had a higher proportion of inconveniences (86.7% vs. 75.9%, p <0.05), and the reference time to pre-transplantation concerning the start of dialysis was longer (median eleven vs. five months, p <0.05). After 2020, the number of first consultations decreased (without significant differences), and the number of patients with problems decreased (51% vs. 86%, p = 0.000). Conclusions: Most patients presented inconveniences during the pre-transplant evaluation process that delayed their admission to the waiting list. The main inconvenience detected was the patient with non-attendance at consultations and/or difficulties in coordinating studies. In the future, it is essential to design strategies that take into account these difficulties and evaluate psychosocial factors and management of the health system.
