La adherencia al tratamiento es un fenómeno común a todas las enfermedades crónicas, que tiene un fuerte impacto tanto a nivel clínico, como sanitario, asistencial y psicosocial. En las personas con esquizofrenia, representa un desafío aún mayor, porque confluyen otros factores cruciales como la comorbilidad con otros trastornos mentales y la falta de conciencia de enfermedad. El presente estudio se propuso estudiar la adherencia al tratamiento y la conciencia de enfermedad en personas con esquizofrenia, indagando sobre los factores que favorecen u obstaculizan el tratamiento. Se realizó una investigación transversal y exploratoria-correlacional, con una muestra discrecional y no probabilística, de 15 usuarios con esquizofrenia que asisten a un centro de rehabilitación psicosocial en Montevideo. Se utilizaron diversos instrumentos de recolección de datos, incluyendo la escala PANSS (estado psicopatológico), DAI-10 (actitud hacia la medicación), SMAQ (adherencia al tratamiento) y SUMD (conciencia de enfermedad). Además, se realizaron entrevistas semiestructuradas y se analizaron historias clínicas para obtener una comprensión más completa de los factores subyacentes. En la población estudiada, se observó una conciencia intermedia con respecto a padecer un trastorno mental (x=2,93 DE=1,2) y los efectos de la medicación (x=3 DE=1,3), y una conciencia baja o muy baja con respecto a las consecuencias sociales del trastorno (x=3,87 DE=1,2) y la conciencia de síntomas (x=4,26 DE=1,0); un 53% de los participantes fueron calificados como no adherentes. Los resultados sugieren la importancia de abordar tanto la conciencia de enfermedad como la actitud hacia la medicación, la relación médico-paciente, la comunicación y la participación activa del usuario, en el diseño de intervenciones dirigidas a mejorar la adherencia al tratamiento en esta población. Estos hallazgos tienen implicaciones importantes para la práctica clínica y futuras investigaciones en el campo de la salud mental.
Adherence to treatment is a common phenomenon in all chronic diseases, which has a strong clinical, healthcare and psychosocial impact. In people with schizophrenia, it represents an even greater challenge, due to the convergence of other crucial factors such as comorbidity with other mental disorders or lack of awareness of the disease. This study aimed to investigate treatment adherence and lack of insight in people with schizophrenia, examining factors that enhance or obstruct adherence to treatment. A cross-sectional exploratory correlational research design was employed, utilizing a judgemental non-probabilistic sample of 15 users with schizophrenia attending a psychosocial rehabilitation center in Montevideo. Multiple data collection instruments were utilized, including PANSS scale (psychopathological state), DAI-10 (medication attitude), SMAQ (treatment adherence), and SUMD (illness awareness). Additionally, semi-structured interviews and clinical record analyses were conducted to obtain a comprehensive understanding of underlying factors. In the studied population, intermediate insight levels were observed regarding experiencing a mental disorder (mean=2.93 SD=1.2) and the effects of medication (mean=3 SD=1.3). However, there was low to very low insight regarding the social consequences of the disorder (mean=3.87 SD=1.2) and symptom awareness (mean=4.26 SD=1.0). Approximately 53% of participants were classified as non-adherent. These results indicate the significance of addressing both insight and medication attitude, the doctor-patient relationship, communication, and active user involvement in designing interventions aimed at enhancing treatment adherence within this population. These outcomes carry important implications for clinical practice and future research in the field of mental health.
