En este artículo se presenta un panorama del tránsito por distintos paradigmas de la salud mental desde una pretendida objetividad centrada en la enfermedad hacia un campo complejo, social y cultural en permanente interpelación, desde un poder centrado en el saber psi, hacia una democratización o un reparto de poderes entre disciplinas y saberes legos, que posicionan la salud mental en su carácter relacional e histórico. Estos tránsitos instalan la pregunta por el sujeto de la salud mental, lo que nos sugiere pasar de su borramiento, enmascarado por la etiqueta diagnóstica y la enfermedad, para asomarnos a la emergencia de un sujeto historizado que, si bien sufre, también resiste a través de su capacidad de agencia y transformación. Este planteamiento resuena con la salud colectiva latinoamericana al considerar el proceso salud-enfermedad-atención-cuidado-muerte como producción social en disputa, que permite nuevos sentidos para la salud mental y sus derivas prácticas.
This article presents an overview of the transit through different paradigms of mental health from a supposed objectivity centered on illness towards a complex, social and cultural field in permanent interpellation, from a power centered on psi knowledge, towards a democratization or a sharing of power between disciplines and lay knowledge, that position mental health in its relational and historical character. These transitions raise the question of the subject of mental health, which suggests that we move from its erasure, masked by the diagnostic label and illness, to the emergence of a historicised subject who, while suffering, also resists through his or her capacity for agency and transformation. This approach resonates with Latin American collective health by considering the health-illness-care-care-death process as a contested social production, which allows for new meanings for mental health and its practical derivations.
