This study examines, through a critical interpretive literature review, how rural populations in Uruguay manage psychological distress by alternating and combining professional psychological services with locally rooted care practices. The problem is framed within the coexistence of public mental health policies, primary care provision, and community networks embedded in rural everyday life, where territorial distance, intermittent access to specialists, and symbolic criteria such as trust, legitimacy, and proximity shape help-seeking decisions. The analysis addresses conceptual and regulatory foundations of mental health in rural and intercultural settings, explanatory models of suffering and local categories, family, neighborhood, religious, and community-based support networks, as well as social representations of psychology and territorial, organizational, and epistemic barriers that influence care pathways. On the other hand, this study takes in consideration the territorial, organizational and epistemic barriers that shape help itineraries. The review identifies tensions in cultural translation between professional languages and community knowledge, alongside experiences of coordination through rural clinics, outreach modalities, and network-based work. On this basis, the study proposes guidelines for culturally responsive psychological interventions grounded in territorial proximity, community participation, intersectoral coordination, and the explicit negotiation of explanatory models within the clinical encounter.
